You are invited to join us to share your views and experiences of the discharge processes on hospital wards, post-discharge experiences of care and the levels of support for carers and carers’ experience during discharge.
This event aims to
- Provide a safe space to share experiences of discharge from hospital
- Help us to improve our discharge process
