Our community stroke team (CST) will help you or a loved one recover and adjust to life following a stroke during the weeks and months after leaving hospital. We will support you, your family or your carer.
It is important you gain your independence – and our experienced team is here to do everything they can to help you achieve this. Our team of physiotherapists, occupational therapists, speech and language therapists, dietitians, psychologist, rehabilitation assistants, specialist nurses and Stroke Association family and carer support officers will care for you. We will support you to cope emotionally and make any necessary adjustments so that you can achieve your goals.
You will be referred to our service from either the stroke unit in the hospital, your GP, social service or other health professional.
Making a referral
Referrals to both the Stockton and Hartlepool community stroke team from the stroke unit at the University Hospital of North Tees are made using a therapy discharge summary and a medical discharge summary to the community stroke team. A copy of the hospital discharge letter or clinic letter should be send to the single point of access for the attention of the community stroke team.
Health professionals can refer by contacting our single point of access or by sending the community services referral form by the email below to make a referral for people with a new confirmed stroke diagnosis.
