Our specialist retinal (eye) screeners, podiatry staff, phlebotomists and support staff will care for you at your annual check up at our one-stop-shop service in One Life Hartlepool.
You will be invited to our clinic once a year if you have type 1 or type 2 diabetes, so we can check your health and see how your diabetes is affecting you.
We will carry out your:
- diabetic eye screening
- foot checks where we will check the blood supply and nerves in your feet
- blood tests
Your information will be given back to your GP so they can talk to you about it at your next appointment with them.
Making a referral
Referrals must be made through the diabetic eye screening service at Lawson Street Health Centre in Stockton.
